Comprehensive Orofacial Myofunctional Evaluation is $250, inclusive of a $50 non-refundable deposit at initial booking.

This Evaluation takes 1-2 hours and is conducted in person on Fridays at our Burien location. Instructions will be provided prior to your reserved time. 

Your Evaluation Report, a detailed narrative report including clinical impressions, recommendations, potential referrals, expectations, therapeutic goals and much more will be provided virtually during your 30 minute follow-up consultation. 

 Once your evaluation is booked, you will receive a registration link. 

Registration must be completed within 24 hours of scheduling in order to secure your appointment. During registration, you will be asked to provide a valid credit card as your form of payment that will be kept securely on file. 

 The $50 non-refundable deposit is charged at time of booking. This will be applied toward the balance of your $250 comprehensive evaluation fee. 

The remainder balance is charged on the day of your appointment. 

Once your Orofacial Myofunctional Comprehensive Evaluation is secured, you will receive several forms to complete.

This step in the process includes Informed Consent Forms, Patient Registration, Intake Forms and Specific Questionnaires related to your case.

• This intake process provides information that is incorporated as part of your overall evaluation.

• These forms must be completed 48 hours prior to your scheduled Comprehensive Evaluation.

This evaluation covers the information detailed above and is the foundation for all treatment.
